How to Apply
Who is eligible to apply?
All applicants must reside in Central Florida and have a household income at 100% of the poverty level. Proof of address and household income is required to submit with application.
What qualifies as dental need?
An incomplete/broken smile, severe noticeable decay or broken or chipped teeth.
Is there a deadline to apply by?
Yes. We accept applications 4 times a year. All applications and required items must be submitted by the dates below to qualify for the process and treatment that quarter.
NOTE: If an application is submitted AFTER the quarters deadline treatment cannot be determined until the following submission date. You will still receive a confirmation of submission but no further process can be started until the next deadline is open.
How do you submit an application?
You can obtain an application by visiting one of the Working Smiles career agency-Career Source of Central Florida. If you are already signed up with a career agency in Central Florida they can also provide you with an application. There you can apply and submit all of the information required.
What do I need to submit with my application?
-Proof of address
-Proof of household income
-Career Assessment (provided by Career Source).
Is there any cost for treatment?
There is no cost to the patient to sign up with a career agency or to apply for the program. There is also no cost for any dental treatment you receive through the program.
How do I know if my application was received?
Once a completed application is submitted it can take up to 2 weeks to get a confirmation of submission.
What happens after my application is submitted?
Once you have received a confirmation of submission, it can take up to 3-6 weeks to set an examination appointment.
What happens at the examination appointment?
A dental examination and x-rays are performed in one of our dental networks provider offices. Your dental treatment and recommendations will be discussed. You will also be required to take a before picture and video testimony. A verbal questionnaire regarding your story and employment goals will also be administered.
How long does this process take?
Once a completed application has been submitted it can take up to 2-3 months from start to finish. This includes the examination appointment, board approval, dental treatment and final follow up.
How do you determine who gets approved?
After your examination appointment, items are submitted to a board of representatives who review your story, dental need and employability and dependent upon the amount of applications and budget a decision is made.
What is required from me once I complete treatment?
After your treatment is completed by one of our network dentists you are required to meet back with Working Smiles for a post op check and follow up. An after picture and video testimony will be needed. Failure to comply with the completion process will nul-invoid your treatment warranty and Working Smiles will not be responsible for future complications with any of the dental treatment provided.
